Male BBL & Buttock Augmentation
Long Island, New York Yee Cheng Low, MD
Proportion and Definition Through Fat Transfer
- 2Weeks without direct sitting
- 4–6Weeks to exercise
- 50–70%Of transferred fat survives

Buttock augmentation with fat transfer improves shape, projection, and proportion using the patient’s own tissue, harvested by liposuction from areas where it isn’t wanted.
For male patients, the aesthetic goal differs meaningfully from the female procedure. The objective is a defined, athletic contour with improved projection and a stronger relationship between the waist, flank, and buttock — not roundness or volume for its own sake. Applying female proportions to a male patient produces a result that reads immediately as wrong.
What the Procedure Involves
Fat is harvested by liposuction — most commonly from the abdomen, flanks, and lower back — then processed and transferred in layers to the buttock. The liposuction itself is half the result: removing fat from the flanks and lower back sharpens the transition into the buttock as much as the transferred fat does.
Safety. Fat transfer to the buttock has been the subject of significant safety scrutiny, and rightly so. Dr. Low places fat exclusively in the subcutaneous plane, above the muscle, in accordance with current multi-society safety guidance. Intramuscular injection is not performed. This is a non-negotiable point of technique.
Benefits
- Improved projection and contour
- Simultaneous liposuction of donor areas
- Uses the patient’s own tissue
- Natural feel and movement
- Improved waist-to-hip relationship
- Permanent, for the fat that survives
Who Is a Candidate?
You may be a good candidate if you:
- Have sufficient donor fat available
- Want improved buttock shape or projection
- Are at a stable weight and in good health
- Can avoid sitting directly on the buttocks for the required period after surgery
Very lean patients frequently do not have enough donor fat for a meaningful result — an honest limitation Dr. Low will raise at consultation.
Recovery & Expectations
The main constraint is positioning: direct pressure on the buttocks must be avoided for approximately two weeks, with gradual return to normal sitting after that. Plan for this before surgery, as it affects work and driving.
Most patients return to desk work at 1–2 weeks with appropriate cushioning, and to exercise at 4–6 weeks. Compression garments are worn over the donor areas.
Expect 50–70% of transferred fat to survive. Volume decreases over the first three months and then stabilizes.
