Facial Fat Grafting

Overview

Much of what reads as facial aging is volume loss rather than sagging. The deep fat pads of the mid-face, temples, and around the eyes deflate over time, hollowing the face and creating shadows that make you look tired and older.

Facial fat grafting restores that lost volume permanently, using your own tissue.

01

What Is Facial Fat Grafting?

Fat is harvested by gentle liposuction — usually from the abdomen or inner thighs — processed, and injected in fine parcels across the areas of loss. Placement in small volumes across multiple layers is essential: fat needs a blood supply to survive.

Commonly treated areas include the temples, cheeks, tear troughs, nasolabial region, jawline, chin, and lips.

Nanofat — highly processed fat, rich in regenerative cells but containing no viable fat structure. Used to improve skin quality, fine lines, and under-eye discoloration rather than to add volume.

  • No risk of allergy or rejection
  • Improves skin quality alongside contour
  • Softens hollowing more naturally than filler
  • Excellent as an adjunct to a facelift
  • Donor area is contoured simultaneously

Fat Grafting vs. Dermal Filler

SettingOperating roomIn-office
Downtime1–2 weeksMinimal

ReversibleNoYes (hyaluronic acid)

Neither is better. Filler suits patients wanting a subtle, reversible, low-downtime change; fat grafting suits patients wanting significant, permanent restoration — particularly alongside other surgery. Dr. Low performs both.

Recovery & Expectations

Swelling is significant for the first week and settles substantially by two weeks, with refinement over two to three months.

Expect 50–70% of transferred fat to survive permanently. The face looks fuller early and settles down. This is expected, not a complication, and a second session is occasionally planned for further volume.
